2026 Varsity National LifeSmarts Championship Team Registration and Competition Agreement

This form is required from all teams that are competing in the National LifeSmarts Championship . You must complete this form in one session. Due by March 5, 2026.

IMPORTANT: NO GUESTS POLICY No students who are not competing are allowed as guests except for invited members of the LifeSmarts LEAP. No parents are allowed as guests unless: 1. Serving as team coach or assistant team coach or 2. Parent who must attend with child due to medical reasons. (this may require a note from student's doctor.) Housing Policy*
National LifeSmarts Championship Attendance and Housing Policy No additional guests (including family members of competitors) are allowed to attend or participate in any portion of the National LifeSmarts Championship. Eligibility for National LifeSmarts Championship Housing A LifeSmarts team is composed of 4 – 5 students plus their coach and assistant coach. Only invited teams may attend. LifeSmarts requires registration from the school-or-organization- approved team coach, team assistant coach (if applicable), and student team members. To accommodate invited teams and for the safety of all attendees, only registered teams are eligible to take advantage of the LifeSmarts-negotiated conference room rates within the LifeSmarts housing block. This includes invited volunteer officials and LifeSmarts state coordinators. Every guest staying within the LifeSmarts conference block must be registered with LifeSmarts. Room reservations will be audited against the official registration roster. Extenuating Circumstances / Medical Need If there are extenuating circumstances, a written request for a waiver may be submitted to the LifeSmarts Program Director. Please acknowledge that you understand the guest policy by typing "YES" below.
Accessibilty Accomodations*
The National Consumers League will make reasonable accommodations for students and coaches with disabilities. If you need special equipment in the hotel (i.e. a special phone for the hearing impaired), contact the hotel directly with your request. We need to know if any students or coaches will be in wheelchairs or using scooters due to room and stage set-ups. Please provide information below. 2026 National LifeSmarts Championship Team Agreement*
We, the undersigned members, agree to participate in all competitions, to attend all assigned matches, all program activities and meals, and to comply with all LifeSmarts rules, policies and procedures. We understand that we will be disqualified and will not receive prizes if we do not participate in all events (including the semi-finals, finals and awards lunch), and /or comply with the rules and guidelines. We understand that we represent our communities, schools, clubs, organizations, and state and will compete fairly and ethically, exhibit good sportsmanship, and interact with other teams, coaches, judges, and sponsors in a way that reflects positively on the LifeSmarts Program.
